The Hampshire Health Record (HHR) is a shared care record which provides a single source of patient information pulled from a multitude of different systems right across Hampshire. It is routinely used by clinicians to share information about nearly two million patients and to support clinical decision-making.
Developed using Graphnet’s CareCentric software, the HHR combines data from general practice and other health and social care providers in many settings across the county. In all, 170 Hampshire GP practices are now signed up to use the system and to upload their data to it. In addition, data is contributed from three acute Trusts, two mental health and community Trusts and three councils. The result is a single electronic patient record which allows clinicians and care workers, no matter where they are based, to access up-to-date information about their patients at the point of care.
