The data stored in HHR is also used to generate an anonymised business intelligence database (HHR Analytics) which supports CCG commissioning and provides a rich source of data for audit and monitoring purposes. Hampshire is using this service in order to develop a detailed benefits analysis of the use of the system.

HHR Overview

Hampshire Health Record Analytics (HHRa)

Hampshire has set up a separate database for research and analytic purposes, the HHRa. This contains pseudonymised data fed monthly from the HHR. The system strips the patient’s name, address, date of birth, and postcode. The NHS number is replaced with an encrypted identifier. The year of birth is kept, and the Index of Multiple Deprivation is assigned from the Super Output Area code.

HHRa uses the following data:

• Hospitals: Three acute Trusts - University Hospital Southampton NHS FT, Portsmouth Hospitals NHS Trust and Hampshire Hospitals NHS FT - feed a number of data streams into the HHR. Data feeds include pathology results, radiology reports, attendances, discharge summaries, outpatient clinical letters and A&E discharge summaries  
• GP practices: HHR receives the bulk of the patient data held in local GP systems including diagnoses treatments, medications, allergies and results. Consultation notes and particularly sensitive data such as IVF and STDs are excluded and the system respects the patient opt out code used by GP systems
• Community and mental health: HHR receives care plans, problems, interventions, medical and social alerts, medications, referrals and summaries from Solent NHS Trust and Southern Health NHS FT. Mental health data is only visible to GPs and senior clinicians and is provided on an opt-in basis
• Social care: HHR receives a range of data including care teams, keyworkers, contacts and other involvements, assessments, needs and care provision details.

How is data held in the HHR accessed?

• The HHR is a web application which is accessed by authorised users from any NHS location known to the system. The system uses comprehensive role-based access methodology and patient consent routines to ensure that access to data can be carefully managed.
• Hampshire has used CareCentric’s ‘embedded’ facility to integrate the product into other systems and offer single sign-on and access within patient context. This means that clinicians at Southampton hospital, for example, can access the shared record from within their PAS and clinical system without having to log on twice or find the patient on two systems. Hampshire has recently introduced the ‘embedded’ facility to EMIS users. This means that a GP can access the shared record for a patient with a single click from the EMIS consultation screen.

• Researchers use data from across the various HHR system feeds and are also able to link in other, condition-specific datasets, such as renal health, cataracts, diabetes, sexual and reproductive health, for further analysis of the quality of care and long term outcomes for certain conditions. Studies have included incidence of post-operative wound infection rates, obesity studies, uptake of diabetic retinopathy and analysis of urinary tract infections in residential care patients.
