· Digital Safety Netting: Automated triage provides tailored advice based on data inputs, including direct links to contact the monitoring team.
· Integrated Records: All information is securely stored within the Shared Care record, offering a complete history accessible to authorised care teams.
In Greater Manchester, the myMaternity app is revolutionising care for women at risk of conditions such as pre-eclampsia and diabetes.
Here are the steps helping to make a big difference:
1. Pre-Eclampsia Detection: Women receive free blood pressure monitors to record readings at home, reducing hospital visits.
2. Diabetes Management: Patients can log blood sugar levels and access their personalised treatment plans via the app.
3. Tailored Care: Clinical teams provide immediate advice based on readings, enabling real-time adjustments to treatment.
This project, part of Health Innovation Manchester, integrates Placental Growth Factor (PlGF)-based testing to enhance pre-eclampsia detection.
The Cheshire and Merseyside Women’s Health and Maternity Partnership has led efforts to address health inequalities in pregnancy since 2015. Their work, heightened by the Covid-19 pandemic, focuses on improving outcomes, access, and services for women in deprived areas such as Liverpool and Knowsley.
One major challenge the Partnership faced was the lack of detailed data on pregnant women’s Covid-19 vaccination rates, including those reluctant to receive the vaccine.
To tackle this, the partnership collaborated with CIPHA (the Combined Intelligence for Population Health Action) and Graphnet to create an interactive pregnancy dashboard. This tool tracks key data, including gestation stage, age, ethnicity, long-term conditions and socioeconomic factors, enabling targeting interventions.
Using these insights, the partnership took the data and worked collaboratively across the system – not just within healthcare but also within communities. The partnership found that effective communication played a key role in increasing vaccination uptake among pregnant women in Cheshire and Merseyside.
