ReSPECT Form

The Recommended Summary Plan for Emergency Care and Treatment (ReSPECT) form empowers individuals to record their preferences for emergency care and treatment, including whether CPR should be attempted.

Although not legally binding, this crucial document ensures a person’s wishes are known and respected when they are unable to express them.

Who Benefits from a ReSPECT Form?

The ReSPECT process can benefit anyone but is especially valuable for:

  • Individuals with complex health needs.
  • Those nearing the end of life.
  • People at risk of sudden health deterioration.

The recommendations on the ReSPECT form are developed collaboratively, involving the individual, their family, and healthcare professionals. This ensures the care plan reflects what matters most to the individual while aligning with realistic clinical possibilities.

Digitised ReSPECT Forms

Graphnet, in collaboration with the Resuscitation Council, has created an accredited digitised version of the ReSPECT form (Version 3.5). This digital plan is integrated within the Graphnet Shared Care Record, providing a secure, accessible, and up-to-date repository for patient care preferences.

The digitisation of the ReSPECT form aligns with the increasing adoption of the process across the UK, enabling faster and more accurate sharing of patient preferences and clinical recommendations within health and social care teams.

Key Features of the Digitised ReSPECT Form

  • Printable Format: Easily share hard copies when needed.
  • Access to Current Preferences: Always work with the most up-to-date patient information.
  • Version History: View and review previous iterations of the form for a complete care record.
  • Enhanced Decision-Making: Enable informed, timely decisions by healthcare professionals during emergencies.

Advancing the Adoption of ReSPECT Across the UK

Graphnet supports the widespread implementation of the ReSPECT process, making personalised emergency care planning more accessible and actionable. By embedding the digitised ReSPECT form into the Graphnet Shared Care Record, we:

  • Streamline the sharing of critical information between professionals.
  • Ensure patient preferences are respected across care settings.
  • Deliver functionality that evolves with healthcare regulations and user needs.

A Commitment to Better Care

The digitised ReSPECT form reflects Graphnet’s dedication to improving patient outcomes and supporting healthcare professionals. By combining cutting-edge technology with a patient-first approach, Graphnet ensures emergency care is personalised, coordinated, and respectful of individual choices.
