Long term condition (LTC) remote patient monitoring enables acute trusts to improve patient care, enhance patient discharge and extend the reach of their services into the community. The technology can also reduce the burden of outpatients.

With around 24% of COPD patients re-admitting within 28 days of discharge nationally, there is a strong case for using patient monitoring to reduce re-admission and support acute trust patients. Evidence from our client studies shows that remote monitoring in COPD could reduce re-admission by 33-50%.

Acute trust patient monitoring can reduce bed occupancy by enabling reduced length-of-stay by physically discharging patients to their home but remaining under hospital care until ready for clinical discharge.
