Frimley Health & Care's Connected Care team and its partners win silver in 2024 HSJ Partnership Awards

Connected Care, part of the Frimley Health and Care Integrated Care System, is delighted to announce that its ‘Supporting Residents with Virtual Care Using Data Driven Digital Transformation’ project won Silver Prize for the ‘Virtual Care Project of the Year’ at the HSJ Partnership Awards, recognising an outstanding dedication to improving healthcare and effective collaboration with the NHS.
Now in its seventh year, 228 entries were received for this year’s Partnership Awards, with 188 projects and individuals meriting inclusion on the final shortlist. The high volume – and exceptional quality – of applications once again mirrors the impressive levels of innovation and care continually being developed across the UK’s healthcare system.
Following a thorough judging process, Connected Care was shortlisted, along with Frimley Health and Care ICS, Graphnet Health, Docobo, Johns Hopkins, East Berkshire Primary Care and Berkshire Primary Care. The awards ceremony was held on 21 March 2024 at Evolution in London.
Connected Care is a dedicated team of people supporting local health and care services to provide a joined-up service across the Frimley Health and Care ICS. They use secure technology, including shared care records and population health management tools, to allow healthcare professionals access to the most accurate and up-to date information, enabling them to make informed decisions about care.
For the winning project, patients living with frailty were offered remote monitoring to help them stay well and receive care if they became ill. The virtual care project team combined clinical expertise with information about a patient’s healthcare needs to proactively offer a remote monitoring service to those most likely to benefit. Technology enabled patients to regularly share their health readings such as symptoms and blood pressure with a dedicated team of healthcare professionals able to respond quickly. As a result, their care could be better planned and offered when needed, reducing the need for patients to be rushed to hospital and improving their experience and ability to live well at home.

We identified residents at highest risk of experiencing exacerbation in illness using a population health-based analytics approach with the collective ambition of supporting them to live and stay well. The remote monitoring service enables patients to know their own baseline and to get advice and care when their symptoms suggest they need it, which helps prevent their condition becoming more serious. Patients are reassured that someone is keeping an eye on their symptoms as well as being more in control of their wellbeing whilst they continue their daily lives at home. Healthcare professionals are able to intervene earlier to help keep people well.
Winning Silver Prize for Virtual Care Project of the Year in the HSJ Partnership Awards 2024 is a stamp of approval for the work that we strive to deliver for our residents and is recognition for the hard work of both our team, ICS and all our partners.
