Graphnet and its partners shortlisted for multiple categories at 2024 HSJ Digital Awards

Graphnet and five of its health and community care partners have been shortlisted across six categories in the 2024 HSJ Digital Awards.

The awards celebrate the power of digital products, projects, and services to transform care delivery, enhance efficiency, and improve patient outcomes.

Graphnet is a finalist in the ‘Improving Medicines Management and Pharmacy Through Digital’ award for its work with NHS Greater Manchester, Health Innovation Manchester, and the University of Manchester on the innovative SMASH population health dashboard.

The SMASH Dashboard prevents prescription errors by running patient data against evidenced-based harm indicators. In the pilot, patients at risk of harm from hazardous prescribing dropped by 40% in 12 months so it was redeveloped within the GM Care Record, giving primary care staff across 414 practices instant access to a standardised, easy to use medication safety tool which allows them to easily review medications across a cohort and make informed, proactive clinical decisions based on the data.

One Wirral CIC and its diabetes prehabilitation service has been shortlisted in two awards - ‘Generating Impact in Population Health Through Digital’ and ‘Supporting Elective Recovery Through Digital.’ The service uses Graphnet Health’s population health platform and analysis of hospital waiting lists at Wirral’s Arrowe Park Hospital to identify and support diabetes patients that are most at risk of having surgery postponed.

Since the programme was launched, One Wirral has seen an average HbA1c drop from 73.4mmols to 63.25mmols over course of 12 weeks, and an average BMI drop from 34.2 to 32.99, amongst the patients they have been supporting.

• Integrated Care Northamptonshire - ICS Digital Team. Digital Team of the Year.

• Integrated Care Northamptonshire - Zeros to Heroes. Digital Organisation of the Year. 

Winners will be announced at an awards ceremony on June 6, 2024, at Manchester Central.
