Every day, over 2.7 million prescription items are dispensed through primary care in England - that’s over 1,900 items every minute [1].
The Safety Medication dASHboard (SMASH) created by the University of Manchester and the Greater Manchester Patient Safety Research Collaboration, is a pioneering system, designed to prevent prescription errors. SMASH highlights patients who are potentially at risk from the medications they are prescribed in general practice to pharmacists and GPs.
The system runs patient record data against evidence-based harm indicators and highlights where reviews are required for patients. SMASH is embedded within Greater Manchester’s Care Record, provided by Graphnet and stores vital health data for 2.8 million patients. This award-winning system provides health teams with a real-time electronic audit and feedback medication safety surveillance system.
