Tameside and Glossop Integrated Care NHS Foundation Trust uses Docobo Remote Monitoring to enable patients to be cared for at home
Tameside and Glossop Integrated Care NHS Foundation Trust works with DOC@HOME® telehealth solution in its Digital Health Service to support 130 patients across the locality. Patients use the Docobo Remote Monitoring CAREPORTAL® handheld devices at home, which enables clinical staff to safely and securely manage them, keeping patients out of hospital where possible and reducing demand on the whole system of care.
The DOC@HOME solution and clinical monitoring is managed by the Digital Health Centre at Tameside and Glossop, with the council team providing the installation and patient training service. The initial implementation took 14 days from kick off meeting to go-live stage – and has now been running for around four years with excellent results.
DOC@HOME is a regulatory compliant digital healthcare management solution for patients in their own place of residence. CAREPORTAL® enables the recording of daily clinical, lifestyle and quality of life data, as well as the delivery of appropriate condition-specific information to educate patients and assist them to manage their conditions more effectively. Collected data and alerts are made available to the Digital Health team at Tameside and Glossop IC NHS FT via the secure HSCN hosted DOC@HOME Web access portal, which provides case management functionality to enable remote management of elderly and vulnerable patients suffering from long term conditions.

We can now support patients at home as an alternative to hospital care, and also support early discharge from hospital, freeing up bed capacity where it is safe to do so.”
